Procedures Performed Deep Plane Facelift, Deep Plane Facelift
This patient sought a deep plane facelift and upper blepharoplasty due to concerns about sagging jowls, lower face asymmetry, and fullness beneath the chin. Her left lower lip has always felt weaker, which she linked to forceps used at birth. Additional areas of concern included redundant upper eyelid skin, marionette lines, visible submandibular glands, and pronounced neck muscles. She had no previous surgeries or cosmetic treatments.
In her surgical plan, I included a deep plane facelift with hyoid fasciotomy and mastoid crevasse to address the facial and neck contour. Her natural hyoid position allowed for optimal support, so we did not need to adjust this structure. Each deep plane facelift includes deep neck sculpting. We had a thoughtful discussion about potential lip lift or buccal fat removal but ultimately chose to focus on the facelift and blepharoplasty alone. She tolerated the procedure well and is now six weeks into recovery. Some swelling remains under her chin, which is typical at this stage and expected to resolve over time. Her brow position stayed untouched, respecting her natural proportions.
