Procedures Performed Revision Rhinoplasty
This patient first sought out a revision rhinoplasty to address specific concerns that had persisted since previous nasal surgeries. After sustaining a nasal fracture at age 16, followed by an initial rhinoplasty and septoplasty, she noticed changes in the structure and function of her nose over time. She described discomfort with the drooping of her nasal tip when smiling, small nostrils, ongoing breathing issues, and a sense of asymmetry. On evaluation, we observed a mild deviation from the midline, a residual dorsal hump, pinching of the nasal sidewalls, and asymmetry in her tip cartilages. The frontal view highlighted a flat nasal tip and some imbalance in projection.
We approached her revision rhinoplasty with the goal to restore both function and balance. Key steps included removal of the dorsal hump, realignment of the nasal bones to improve symmetry and correct the pinched appearance, and placement of an extension graft for tip support. Adjustments to her tip cartilages were performed to refine the nasal contour and subtly increase projection. Our aim was to improve both the appearance and the airway, enhancing overall nasal harmony.
Throughout the decision-making process, we maintained open communication about each surgical step. We shared realistic expectations and individualized the operative plan to address her unique anatomy and concerns. We are pleased with her progress in recovery, which shows improved tip definition, symmetry, and airway function.
